Field Trip Request Packets

  • You, or an M-DCPS employee from your school site that you designate, must be present at all in-person auditions, rehearsals, and concerts that include your students.

  • You must complete field trip forms for your students to participate in rehearsals, concerts, and in-person auditions, and bring the forms to each event. If you are designating a different M-DCPS employee from your school site to attend, enter their name as Name of School Group Sponsor. We have created a pre-populated template for your convenience.
